How to Apply
Welcome to UP Visayas!
This is your first step toward admission to the University. To ensure your application is processed smoothly, please follow the steps below carefully.
Documentary Requirements
Please scan hard copies of documents (except for pictures) and save as PDF with corresponding file names. Upload scanned documents (in pdf) at the designated bins.
NOTE: You are required to submit your documents both online and in person (or via courier) to the UPV Graduate School.
Required Documents
A
Accomplished application forms and information sheets with passport size or 2×2 photo. Please access the prescribed form by clicking on the button
B
Undergraduate transcript of academic records (including graduate and post-graduate, if applicable);
D
Certificate of Honorable Dismissal/Certificate of Transfer Credential/ Certificate of Transfer Eligibility from the latest school attended (applicable only to non-UPV graduates and to UPV graduates who have enrolled in other school/s); affix student’s signature in the space provided (if required in the CTC);

Original Birth Certificate issued by the National Statistics Office (NSO) or Philippine Statistics Authority (PSA);
F
Original Marriage Certificate issued by the National Statistics Office (NSO) or Philippine Statistics Authority (PSA) (applicable only to married female applicants);
G
Copy of the official receipt or proof of payment of NON-REFUNDABLE application fee (local applicant – ₱250.00; foreign applicant – US $20.00); please click on the button to view the payment options and instructions
H
Two (2) letters of recommendation from former professors and/or immediate supervisors (referee to accomplish the online form). Please copy this link and forward to your referee
Recommendation letters can also be emailed by the Referee directly to gs-secretary.upvisayas@up.edu.ph, or hard copies inside sealed envelopes may be submitted to the Graduate School office

Two (2) passport- size pictures
In addition to completing the online admission application, you are required to submit the ORIGINAL hardcopies of the necessary documents to the UPV Graduate School.
You may submit these documents (in a long white folder) either:
- In person at the UPV Graduate School office, or
- Via courier to the following address:

NOTE: The Transcript of Records submitted is for evaluation purposes only. The official TOR from your school (with the remarks “Copy for UP Visayas”) will be required by the UPV Registrar after admission. This is a school-to-school transfer of credentials.
